In collaboration with the Office of Student Health Services, the School District of Philadelphia is pleased to announce that this summer, CityLife Health will be providing students with the opportunity to receive both school-required immunizations and physical exams (including mandatory school entry and PIAA sports physicals).
Students in 6th through 12th grade will be able to receive immunizations and physicals at select middle schools and high schools hosting School District Summer Programming, whether or not they are enrolled in a summer program.
All students, K-12th grade, can also receive all of these services at any CityLife Health Clinic in Philadelphia. These services will be provided to all students at no charge, regardless of insurance status. (CityLife Health may seek reimbursement from Medicaid for services to such students covered by Medicaid)
Families can schedule an appointment for their child with CityLife Health by registering online at www.philasd.org/vax or by calling 1-877-926-9272. Students under 18 must have parental consent. Student-Athletes must have their correct PIAA forms with them
Walk-ins are welcome on the day of the event but appointments are preferred.
